The drug is a remedy for the treatment of urolithiasis. The medicine is used – for metaphylaxis of calcium stones, mainly oxalate stones (prevention of recurrence or increase in the size of the remaining fragments), for alkalization of urine in uric acid stones, even in the presence of calcium stones, to enhance the effect of the drug, patients should be advised to reduce salt intake and increase daily fluid intake during treatment. Active ingredients – 159 mg sodium carbonate, 980 mg sodium bicarbonate, 1409 mg citric acid, 1586 mg potassium citrate-1-water. Excipients – natural flavors identical to natural, Povidone K30, 100 mg sorbitol, sodium cyclamate. The medicine should not be taken if you have hypersensitivity (allergy) to citric acid, potassium citrate, sodium bicarbonate, sodium carbonate or any other component of the medicine, with impaired renal function, with severe liver dysfunction, with metabolic alkalosis, with chronic urinary tract infections with urea-splitting bacteria (risk of struvite stones), severe hypertension, on a low-salt diet, and with Adynamia Episodica Hereditaria (periodic hyperkalemic paralysis; a rare hereditary disease with paralytic symptoms). Always take this medicine exactly as prescribed. Unless otherwise prescribed by the doctor, the usual dose is: The daily dose should be adjusted individually for the patient so that the pH of freshly collected urine is 6.0–7.0 before taking the next preparation (for pure uric acid stones the ideal pH is 6.2–6.8, and for calcium stones 6.4–7.0). For adults, the following dosage recommendations are given: a) 3–4 effervescent tablets daily, divided into 3 doses – corresponding to 9.9–13.2 g of potassium sodium citrate per day (i.e. 90–120 mmol of alkali or 36–48.8 mmol of citrate): 1 effervescent tablet should be taken in the morning and at midday, and 1–2 effervescent tablets in the evening. b) In the case of calcium stones, 1 to 2 effervescent tablets can also be taken in the evening as a single dose, which corresponds to 3.3 to 6.6 g of potassium sodium citrate (i.e. 30 to 60 mmol of alkali and 12 to 24 mmol of citrate). To ensure adequate dosing, it is recommended to measure the amount of citrate excreted over 24 hours as well as the urine pH value at the beginning of therapy and at each dose adjustment. If the specified pH range is exceeded or not, the daily dose should be increased or decreased. This adjustment should preferably be made in the evening. If you have taken more than you should: Cases of overdose with potassium sodium citrate have not been described to date. With adequate renal function, undesirable effects on metabolic parameters – even when the recommended dose is exceeded – are not to be expected, since the excretion of excess bases by the kidneys is a normal regulatory process for maintaining the acid-base balance. Avoid exceeding the stated urinary pH range for several days, as this increases the risk of calcium phosphate crystallization on the one hand, and long-term metabolic alkalosis should be avoided on the other. A possible overdose can be corrected at any time by reducing the dose. If necessary, measures to treat metabolic alkalosis should be considered. If you forget to take: Do not take a double dose. Side effects, Mild gastrointestinal disturbances (stomach discomfort, nausea, vomiting and mild diarrhea) are common with potassium sodium citrate. Very rare: hypersensitivity reactions.
